Admissions

When you arrive for admission, please bring the following:

• Picture ID
• Insurance cards
• Medicare/Medicaid cards
• Your current medications
• Advance Directives
  • Living will
  • Healthcare power of attorney
• Discharge arrangements
  • Who will pick you up from the hospital?
  • Where you will go after you leave the hospital?
• Emergency contact information
• Any information from your physician (i.e. admission orders)
• Personal items (pajamas, slippers, robes, etc.)

Please leave all patient valuables at home or with a loved one.
